HealthForms is helping school nursing services identify and respond to the health and wellbeing needs of children and young people, through digital screening.
With more than 46,000 forms completed across six school year groups in the 2025-26 academic year, it’s enabled school nursing teams across the NHS to understand population health needs and offer direct support where it’s needed most.
Backed by NIHR research, HealthForms continues to demonstrate the value of digital approaches to early identification and prevention in line with service delivery of the Department of Health & Social Care’s Healthy Child Programme.
Other NHS teams are also using HealthForms to deliver digital waiting list reviews for children and young people in services such as CAMHS, speech and language therapy and continence services.
