Impact Report 2025-26

This year has been one of growth, collaboration and impact for the Digital Health Transformation Service, with ChatHealth, HealthForms and HealthWebsites continuing to support NHS organisations to improve access to healthcare and enhance service user experience.

Over the past year, we have extended our reach across the NHS, enabling more services to connect with service users, families and communities through accessible, digital-first solutions. Our growing community of practice has brought together clinicians, service leaders and digital experts to share learning, innovation and best practice, helping to maximise the value and impact of digital healthcare.

As the NHS pursues the ambitions of the 10 Year Health Plan, we remain committed to supporting more preventative, person-centred and digitally-enabled care. By working in partnership with healthcare teams, we are helping to create sustainable improvements in access, experience and outcomes.

This report showcases the achievements, partnerships and insights that have defined a successful year and highlights the positive difference ChatHealth, HealthForms and HealthWebsites are making for services and the communities they serve.

Through secure messaging, digital forms and trusted online information, our services are supporting people to access advice, support and care in ways that are convenient, inclusive and responsive to their needs. This not only improves engagement but also helps services work more efficiently and reach those who may otherwise face barriers to accessing care.

Improved Outcomes

ChatHealth Staff User Survey

  • Respondents represented 46 NHS Trusts and healthcare organisations.
  • 94% use the ChatHealth Staff Portal with almost half using it weekly as part of routine service delivery.
  • 87% work in services that have used ChatHealth for more than 12 months.

Healthcare Professionals reported their messaging services are helping to support earlier intervention, increase efficiency and deliver care through safe, accessible digital communication.

Healthcare professionals highlighted the importance of promotion in maximising the reach and impact of ChatHealth through consistent promotion, clear communications and staff advocacy.

HealthWebsites Staff User Survey

  • Respondents represented five NHS Trusts and healthcare organisations.
  • Over half of respondents have permissions to create, publish and manage content within their Local Area pages.
  • Respondents managed Local Areas across all three websites – Health for Teens, Health for Kids and Health for Under 5s.

 

Impact on NHS Services

Healthcare professionals reported that the suite of health promotion websites strengthen local service delivery, support workforce practice and improve access to trusted health information for children, young people and families.

Healthcare professionals identified promotion as the greatest opportunity to increase awareness and engagement with Local Area pages.

HealthForms Staff Survey

  • Respondents represented seven NHS Trusts and healthcare organisations.
  • The majority of respondents can use the HealthForms Dashboard to view form responses, red flags and reports.
  • HealthForms questionnaires were completed across six school year groups, from Reception through to Year 13.

 

Impact on NHS Services

Healthcare professionals agreed that HealthForms supports earlier identification of needs, safe disclosure of sensitive information and more efficient delivery of school health assessments.

Engagement and Participation

Healthcare professionals drive participation in HealthForms through proactive engagement activities, including school partnerships, leadership support, effective communication and visible school nursing presence.

  • “Service users can access us on the go, at a time that suits them. ChatHealth reduces the need to wait for health information and advice, while providing timely access to follow-up support if needed.”

Quality Assured Care

Robust processes are behind all our products to ensure that high levels of clinical safety and technical security is central to our work. We are continually assessed and evaluated to meet NHS clinical safety standards and technical quality and security standards.

Risk management

We regularly review our compliance with the information standard ‘Clinical Risk Management: its Application in the Manufacture and Use of Health IT Systems’. This assures how we identify and mitigate clinical risks, document processes for clinical safety and test for issues that could affect clinical safety.

Clinical Quality

We support our user organisations and give them practical tools to deliver high quality care.  We do this through structured documentation, high standards and expectations for implementation and the sharing of best practice, alongside the creation of national networks and communities of practice.

Technical security

All our digital tools are supported by a highly-rated technical support team. Information governance standards are assured by our Data Protection Impact Assessment (DPIA) processes, and we adhere to all NHS related compliance frameworks. Our service is part of Leicestershire Partnership NHS Trust which has met the standards of NHS England’s Digital Data Security and Protection Toolkit (DSPT). We also have evidence ready to share with prospective organisations assessing our products against NHS England’s Digital Technology Assessment Criteria (DTAC).

Strong Partnerships

Our partnerships with user organisations are formed at the start of their implementation. Lifelong support is offered to help services deliver a successful digital offer. This includes help desk support, annual contract reviews and ongoing guidance on technical, marketing, or clinical aspects as required.

Supported Implementations

Highly structured and well-refined implementation processes have been developed by our PRINCE2 certified project manager. Our effective training programmes have been created by our TAP Learning qualified trainer. This ensures our implementation processes are as straightforward and efficient as possible for user organisations to adopt ChatHealth, HealthWebsites or HealthForms.

  • Dedicated support for the key worksteams covering clinical, marketing and technical aspects
